If you haven’t completed New Patient Form online refer back to New Patients process to establish online access) Create a profile with user name (user name is your email on file with our office). A code will be sent to you via email or text to confirm identity to complete your initial profile set up. You will have access to update health history changes on current health history form, update patient information, review account, request appointment, access to secure email.
